The services you receive from CCM will be individualized to your personal needs. Once you are enrolled, you will receive all your medically necessary health services through CCM. Working with you and your family, a team of physicians, nurses, social workers and therapists will develop a plan of care for the services you need, which may include, among others:

• Primary and Specialty Medical Care, either at CCM centers or in the community
• Nursing Services at our centers and at home
• X-rays and other Laboratory Services
• Medications
• Medical Equipment
• Hospital Care
• Transportation to and from our centers and medical appointments
• Emergency Medical Services around the clock
• Occupational and Physical Therapy, to improve your ability to function as independently as possible
• Speech and Hearing Therapy
• Therapeutic Recreation including social discussion groups, arts, crafts, parties, cultural and education programs at our cheerful day health centers
• Social Work Services including supportive counseling, help with benefit entitlements, and other assistance you may need
• Nutritious Meals in our day health center
• Personal Care Aides in the home to assist you with meals, shopping, and other help you may need
• Home Modifications to accommodate your disabilities and special needs
• Support for Family Caregivers
• Nursing Home Care
