CCM Eligibility

You may be eligible to join CCM if you meet the following criteria:

* Are 55 years of age or older

* Reside in Manhattan, the Bronx, Brooklyn, Queens or Westchester County

* Are Medicare and Medicaid (or just Medicaid) eligible, or are willing to pay privately for services

* Are nursing home eligible (frail, disabled, and/or mobility impaired)

* Are able to live safely in a community setting

* Require 120 days or more of long-term care services.
