CCM's innovative
program of health services allows our members who would otherwise
require nursing home care to stay in the community as long as
possible and receive high quality, comprehensive care.
A
program developed especially for you
Everyone's
health care needs are unique. CCM and our team of physicians,
nurses, social workers and therapists work with you and your
family to tailor a program that's just right for you.
Your individual
health care plan will provide for all your health care needs.
Our goal is to improve your health and quality of life so that
you may live as independently as possible.
Your
home away from home: CCM's Day Health Center
Once you
become a CCM member, you may enjoy the recreational activities
offered at our cheerful Day Health Centers. Door to door transportation
will be provided.
At our Centers,
you can make new friends, socialize, catch up on local events,
and participate in a wide range of stimulating, fun activities.
You will
enjoy nutritious meals here that meet your personal dietary
needs.
While you
are here, you will also receive a variety of health care services
such as therapy, social work services and medical care, to keep
you as healthy as you can be.
Living
at home with the help you need
When you
are not at a CCM Center, you will receive the extra help you
need to stay safe and comfortable at home.
In addition
to your nurse visiting you at home you may also receive trained,
supervised staff to help with personal care, shopping, meal
preparation or simple household needs, nursing care, or even
modifications to your home to accommodate your disabilities.
Coordinated
care that meets your personal health care needs
Over time,,
your medical and social needs may change. Your CCM team monitors
your health and well-being, and will make any changes needed
to your plan of care, to keep you as healthy and independent
as you can be.
The CCM
team will provide the support family caregivers need so that
they may continue to assist you to remain in your own home.
