CCM Direct has a variety of plans available. Please refer to the table on the right for more information. These plans are available in the following counties of New York State: Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Suffolk, and Westchester. Please note that this service area is subject to change if CCM's contract with Medicare is terminated in one or more counties. We will tell you in advance if this happens.
