Welcome to CCM Direct, a Medicare-approved health plan designed to meet your medical and prescription drug needs.CCM Direct members have low or no monthly premiums; access to experienced primary care physicians and specialists; a network of leading hospitals in our service area; coverage for Medicare Part D prescription drugs; vision and dental care; free health screenings for many conditions; and emergency room coverage wherever needed in the United States. CCM Direct contracts with the Federal government to provide these services. CCM Direct also has contracts with pharmacies that equal or exceed CMS requirements for pharmacy access in your area.
