How to Enroll in CCM Direct

CCM Direct can enroll members who meet the follow three criteria:

  1. You live in the service area approved for CCM Direct: Manhattan, Bronx, Brooklyn, Queens and Westchester. Other important information for members can be found in CCM Direct's Summary of Benefits.
  2. You are entitled to Part A of Medicare and are also enrolled in Part B of Medicare, and you continue to pay your Medicare Part B premium; or you are entitled to Medicaid and Part A of Medicare and are also be enrolled in Part B of Medicare, and you continue to maintain your Medicaid benefits and pay your Medicare Part B premium.
  3. Individuals with end-stage renal disease (ESRD) are not eligible to enroll in CCM Direct. An individual who receives a kidney transplant and who no longer requires a regular course of dialysis to maintain life is not medically determined to have ESRD.

If you meet these requirements, enrollment is easy. Just call CCM Direct Customer Service at 1-877-CCM-8500 and we will schedule an appointment to visit at your convenience. TTY/TDD users please call 1-800-650-2774. Customer Service representative are available seven days a week, from 8:00 a.m. to 8:00 p.m.

CCM Direct is a Coordinated Care plan with a Medicare Advantage contract. You must receive all routine care from network providers. If you obtain routine care from out-of-network providers neither Medicare nor CCM Direct will be responsible for the costs. Please contact CCM Direct for details.
