How to Enroll in CCM Direct

CCM Direct can enroll members who meet the follow three criteria:

  1. You live in the service area approved for CCM Direct: Bronx, Kings (Brooklyn), New York (Manhattan), Queens, Richmond (Staten Island), Nassau, Suffolk, and Westchester counties. Other important information for members can be found in CCM Direct's Summary of Benefits.
  2. You are entitled to Part A of Medicare and are also enrolled in Part B of Medicare, and you continue to pay your Medicare Part B premium; or you are entitled to Medicaid and Part A of Medicare and are also be enrolled in Part B of Medicare, and you continue to maintain your Medicaid benefits and pay your Medicare Part B premium.
  3. Individuals with end-stage renal disease (ESRD) are not eligible to enroll in CCM Direct. An individual who receives a kidney transplant and who no longer requires a regular course of dialysis to maintain life is not medically determined to have ESRD.

If you meet these requirements, enrollment is easy. Just call our Member Services Department at 1-877-226-8500 and we will schedule an appointment to visit at your convenience. TTY users please call 1-800-650-2774. Member Services Representatives are available seven days a week, from 8:00 a.m. to 8:00 p.m.

CCM Direct is a Coordinated Care plan with a Medicare Advantage contract. You must receive all routine care from network providers. If you obtain routine care from out-of-network providers neither Medicare nor CCM Direct will be responsible for the costs. Please contact CCM Direct for details.
