Comprehensive Care Management Corporation (CCM), a not-for-profit organization, is a New York State Managed Long Term Care Plan and a federal Program of All-Inclusive Care for the Elderly (PACE).

CCM’s innovative health care program allows frail elderly and disabled adults to stay in their own homes for as long as possible while they receive high quality, comprehensive care.
