CCM Select is a comprehensive long term care managed care program for adults who need assistance with daily activities and with the necessary services, can live safely at home.

CCM Select members benefit from the coordination of care provided by a CCM Select Nurse Care Manager, who works with you, your family and your physician to develop a plan of care that is right for you.

Covered Services may include the following:

• Care Management
• Nursing Home Care (see section below for limitations of coverage)
• Home Care (nursing, home health aide, physical therapy, occupational therapy, speech pathology, and medical social services)
• Adult Day Health Care
• Personal Care
• Durable Medical Equipment, Medical/Surgical Supplies, Prosthetics and Orthotics
• Personal Emergency Response System
• Non-emergent Transportation to and from your scheduled health-related appointments
• Podiatry
• Dental Care
• Optometry Exams and Eyeglasses
• Physical Therapy, Occupational Therapy and Speech Therapy or other therapies provided in a setting other than a home
• Audiology Exams and Hearing Aids and Batteries
• Respiratory Therapy, including preventive, maintenance and rehabilitative airway-related techniques and procedures
• Nutrition
• Private Duty Nursing
• Home Delivered or Congregate Meals
• Social Day Care
• Social and Environmental Supports, including home improvements and modifications, appliances and assistive devices

Non-Covered Services
Non-covered services include physician services, hospital services and others (complete list is available). These non-covered services are paid for by Medicare and/or Medicaid fee-for-service. Members are free to select providers of their choice for non-covered services.

Members' physicians are required to collaborate with CCM Select and the Member on developing and coordinating a Plan of Care. Only services included in the Plan of Care will be covered by CCM Select.

You may be eligible for CCM Select if you are:

• 18 or older
• A resident of Manhattan, Brooklyn, Queens, Bronx or Westchester County
• Medicaid eligible
• Nursing home eligible, able to live safely at home at time of enrollment, and require care management and the long term services of the plan for 120 days or more
